How long do dental implants really last? An honest answer

  • Patients ask us this almost every week, usually right after a relative's implant "failed" or a YouTube video promised "implants for life". The honest answer sits between those two extremes.
  • ## What the numbers actually say
  • Well-placed implants in healthy patients survive beyond 20 years in the large majority of cases — long-term studies consistently report survival above 90% at the 10-year mark. But survival is not the same as trouble-free. The crown on top of an implant is a mechanical part; like any mechanical part it can chip, loosen or wear, and a crown replaced at year 12 does not mean the implant failed.
  • ## What actually shortens an implant's life
  • Smoking. Nothing else on this list comes close. It starves the gum and bone of blood supply exactly where the implant needs it.
  • Uncontrolled diabetes. Well-controlled sugar levels are fine; an HbA1c that swings wildly is not.
  • Skipped hygiene visits. Implants don't get cavities, so patients relax — and then peri-implantitis, a gum infection around the implant, does the damage a cavity would have.
  • Night grinding. If you clench or grind, an unprotected implant crown takes forces it was never designed for. A night guard is cheap insurance.
  • ## Why we sometimes say no
  • At our clinic the assessment matters more than the placement. We look at bone volume on the X-ray, gum health, bite forces and your medical history before agreeing to place anything. Occasionally we advise a bridge or even a well-made denture instead — an implant placed into a mouth that can't maintain it helps nobody, least of all you.
  • ## The part most people skip
  • The single best predictor of a 25-year implant is boring: a professional cleaning and check every six months, and a patient who treats the implant like a natural tooth rather than an indestructible one. That's it. No secret titanium, no premium mystery brand — maintenance.
  • If you're considering an implant, bring your old X-rays if you have them. The more history we see, the more honest our answer can be.
